BS AU 159 draws a line across the tyre, and everything is decided by which side of it the hole sits. The central three quarters of the tread can be repaired. The shoulders and the sidewall cannot, because that is the part of the tyre that bends on every single revolution and no patch survives being flexed like that. It is not a judgement call and it does not change at three in the morning.
So the first thing that happens at the kerb is that the tyre comes off the rim and gets looked at from the inside. If the object went in within the repairable band, the hole is reamed to size, a combined plug and patch goes in from the inside, and the wheel goes back on the balancer. If it went in at the shoulder, we say so straight away rather than sealing it and letting you find out about it at speed.
Two in the morning is a bad time to discover a flat and a worse time to try and find help. Every time. The entry point tells you very little from outside. The inside of the casing tells you whether the tyre has been run soft, whether the object tracked sideways as it went in, and where it actually came through.

It happens more than you would think. Sometimes the leak is a perished valve, sometimes it is the seal where the tyre meets the rim. We find it with soapy water rather than assuming, and the fix is different in each case.
