The distinction that decides everything is tread against sidewall. A clean puncture in the central three quarters of the tread, up to about six millimetres across, can normally be repaired properly from the inside. Anything in the sidewall or the shoulder cannot, whatever the sealant tin claims, because that part of the tyre flexes several times a second and no patch will stay bonded there.
A bulge is not damage to the rubber at all. It is the casing cords underneath having snapped, with only rubber left holding the pressure at that point. That tyre is finished.
