By the time somebody rings and calls a tyre broken, they have usually already decided it is finished, and about half the time they are wrong.
Start with where the damage is. A hole in the flat band that meets the road is a puncture and typically fixable. A slice, gouge or split in the flexible wall between tread and rim is not, because that section flexes thousands of times a mile and no patch will stay bonded there. Between those two is the shoulder, the curved corner, and damage there falls on the wrong side of the line as well.
Then look for the things that mean the structure has failed rather than been punctured: a bulge, exposed cords, tread lifting from the casing, or a rim with a fresh flat spot beaten into its edge from an impact.

Not always. It has to be within the central band, no wider than six millimetres and not so deep that it has cut the steel belts. A long slash across the tread, even in the right place, is usually beyond what a patch is designed to hold together.
It means the fabric or steel structure under the rubber is showing, and the tyre has no reserve left at that point. It is a fail on an MOT and a genuine risk on the road. There is no repair for it; the tyre needs changing.
