A nail found in the tread is one hole, and one hole in the right place is repairable. The error is assuming there is only one. Something long picked up at low speed rarely goes straight in and stops. It goes in at an angle, travels under the tread and comes out again, sometimes through the far side of the crown, sometimes through the shoulder, occasionally back out of the same face a few inches along. From the outside you see a head and nothing else. Everything that actually decides the outcome is on the inside.
That is why the tyre comes off the wheel before anybody says the word repair, and why the liner gets looked at all the way round rather than just around the obvious injury. Two holes matter for a specific reason: each patch needs sound cord around it, so two injuries close together cannot both be served, and an exit through the shoulder ends the conversation regardless of how tidy the entry looked. It takes a couple of minutes to check properly. It is also the step that stops a tyre being handed back, inflated and reassuring, with a second hole still in it that will show up in a fortnight.

By entering at an angle. Objects lying flat on the road get flicked up and driven in sideways rather than straight down, so they travel under the tread for a distance and emerge somewhere else. The entry and exit can be several inches apart and only one of them is usually visible.
Sometimes, if both sit within the repairable band and there is enough undamaged casing between them for each patch to bond to properly. Close together, no. The patches would overlap or share weakened cord, and at that point neither of them is doing the job it exists to do.
Not on its own. An object still sitting in the hole plugs it well enough to keep the pressure up, which is why plenty of tyres with two injuries feel entirely normal. The pressure tells you the hole is currently blocked, not that the casing underneath is sound.
