The head of a nail tells you where it went in. It tells you nothing about where it stopped. Objects do not enter a rotating tyre squarely. They are struck at whatever angle the tread happens to present as the wheel comes over them, and a long thin one that goes in leaning can then run at a slant between the steel belts and the tread rubber before its point finally comes through the casing. We have taken out nails that entered in the middle of the tread and were doing their damage under the shoulder, a good couple of centimetres from where anybody looking at the outside would have placed them.
That gap between the entry and the exit is why an assessment made from the pavement is a guess. The rule that decides a repair is about where the casing is actually punctured, and the shoulder is excluded because it flexes harder than anything else on the tyre, so a patch out there gets peeled rather than pressed. The tyre therefore comes off the wheel, the object comes out with the path it took visible behind it, and the position that matters is measured from the inside. Only then is there an answer worth giving, and you get it either way rather than after the work has already started.

Not on its own. A slanted entry that still ends up inside the central part of the tread gets repaired normally. What rules it out is the exit landing in the shoulder, or a channel so long and torn that there is no clean hole left to fill properly.
